How to apply for a CCNAPI Membership Online with an electronic Membership Card?

(For Year 2022)

 

PREPARE ALL THE REQUIREMENTS:

  • Current Copy of PRC License - (must be in JPG)
  • Current valid Copy of PNA membership card (must be in JPG)
  • ID picture - 1 x 1 or 2 x 2 (must be in JPG)
  • If Employed (working in MS Area, ICU, PACU, CCU, PICU): copy of certification employment currently working for not less than 3 months, signed by your immediate supervisor (must be in JPG or PDF) 
  • If not currently employed: copy of any certificate/s with accumulated 20 credit     

units from any CCNAPI training attended within 1 year. (must be in JPG or PDF)

  • Deposit slip payment of Php 500.00 (must be in JPG)

MEMBERSHIP APPLICATION

 

Steps how to Avail:

 

  1. Create your own account by clicking REGISTER NOW (make sure to keep the details of your account)
  2. Fill out the necessary requirements. Click NEW or Renewal.
  3. If there are fields that are not applicable, please type NA. For the year that is NA, put 2022.
  4. Make sure all requirements should be in JPEG format For COE / Certificates you may jpeg or PDF (if multiple files).
  5. Click REGISTER, OTP VERIFICATION PLEASE CHECK YOUR REGISTERED EMAIL. (kindly check your inbox or SPAM folder)
  6. Once register you will be redirected to the log-in page. Kindly put your Username and password.
  7. click the AVAIL BUTTON then choose, click AVAIL MEMBERSHIP or AVAIL ITEM (applicable for CCNAPI Courses)
  8. the 1 YEAR MEMBERSHIP (500.00 fees for now), then Click on the Action “+” button to add to Cart. Then click CONFIRM BUTTON
  9. Click the Pay Now and upload the copy of your deposit slip (jpeg file) chose PTBD or online payment)

ALL MEMBERSHIP and COURSES PAYMENTS SHOULD THRU SECURITY BANK ONLY.

  1. Check if your status is for APPROVAL/
  2. Kindly wait for approval of your membership within 5 working days. Try to log in again to your account if our application has been approved.
  3. Once approved,  you may save your electronic card (in PDF format) or print it.
